Vaccine requirements for Boarding, Grooming and Day care.
At Dworkis Dog & Cat Hospital, we strive to maintain the health and well being of your pet when they are here for a visit. Our hospital policy requires both annual and semi vaccines for any boarding, grooming and day care pets. If the services were not performed at our facility, proof of vaccination and current flea and heartworm preventative MUST be presented at time of drop off.

Bordetella Leptospirosis Rabies Current negative Fecal Test Current Heartworm Test Flea and Heartworm preventative (veterinary approved. Pet store products are inadequate)
DHLPP ~ Dogs
- For Cats:
- Feline Distemper Complex
- Feline Leukemia (If cat goes outdoors)
- Rabies
- Current Heartworm Test
- Current negative Fecal Test Flea and Heartworm preventative (veterinary approved. Pet store products are inadequate)
FVRCP ~ Cats
